Crown Lengthening
Crown lengthening is a procedure performed to enhance the health of the gum tissue, prepare a tooth for restorative treatment, or improve the appearance of a “gummy smile.” A gummy smile occurs when excess gum tissue covers too much of the teeth, creating an uneven or less attractive smile.
This treatment involves carefully reshaping and recontouring the gum tissue — and in some cases the underlying bone — around the affected tooth or teeth. The goal is to establish a healthier and more balanced gum-to-tooth relationship. Crown lengthening may be completed on a single tooth, multiple teeth, or across the entire gum line, depending on the patient’s needs.
When Is Crown Lengthening Needed?
Crown lengthening is commonly recommended when a tooth requires a new crown or another type of restoration, but the edge of the restoration extends too far below the gum line. When this margin is too close to or beneath the bone, it becomes difficult to properly place and secure the restoration.
By performing crown lengthening, we can expose more of the natural tooth structure. This ensures the new restoration fits properly and remains secure over time. It also makes it easier for you to maintain good oral hygiene around the restoration, helping to prevent decay and gum disease.
What to Expect During the Procedure
The procedure typically takes about one hour, although the exact time depends on how many teeth are involved and whether bone reshaping is necessary. Crown lengthening is usually completed under local anesthesia to ensure your comfort.
Small, precise incisions are made to gently separate the gum tissue from the teeth. Even if treatment is required for only one tooth, nearby teeth may also be adjusted to create a smooth and natural-looking gum contour. In some cases, a small amount of bone may be removed to achieve the desired result.
Once Dr. Michael Strassberg, DDS, Dr. Juliana Ko, DMD, and Dr. Milita Borguet, DMD determine that sufficient tooth structure has been exposed and the reshaping is complete, the area is thoroughly cleaned. Sutures and a protective dressing are then placed to support proper healing and stabilize the new gum-to-tooth relationship.
Recovery and Healing
Immediately after the procedure, your teeth may appear longer due to the repositioned gum tissue. A follow-up appointment will typically be scheduled within one to two weeks to remove sutures and monitor healing progress.
Complete healing of the surgical site generally occurs within two to three months, after which your final restoration can be placed if needed.
