Welcome to Mainline Dental Arts! Your first visit is an important step toward achieving and maintaining excellent oral health. Our team is committed to making your experience comfortable, informative, and stress-free.

To help us prepare for your appointment, please bring the following:

Any referral information, if applicable
A list of medications you are currently taking
Completed patient registration and medical history forms (these may be filled out online prior to your visit)
Your dental insurance card and any necessary insurance forms
A valid driver’s license or photo ID

Please Note: Patients under the age of 18 must be accompanied by a parent or legal guardian at the consultation appointment.

What to Expect During Your Appointment

During your first visit, we will:

Review your complete medical and dental history
Take any necessary digital X-rays
Perform a comprehensive oral examination
Evaluate your teeth, gums, and bite
Screen for oral cancer
Assess overall oral health and discuss any findings

Our doctors will take the time to explain your oral health status and discuss personalized treatment recommendations, if needed. We encourage you to ask questions — we are here to ensure you feel confident and informed about your care.
